I/we hereby release and waive all claims against United Cerebral Palsy (UCP), UCP sponsors, vendors or any employees for any injury or illness that I/we might suffer while participating in or from this event. I/we attest that I/we are physically fit and prepared for this event. I/we further authorize the use of the name, contact information, stories, images or other information about myself and my child for media, marketing and other UCP promotional and/or fundraising purposes. I/we understand I/we have the right to revoke this option in writing to UCP and that my information and images not be sold, donated or rented to a third party without my expressed written permission. If Participant is a minor, the parent or guardian must agree to the below: I am the legal guardian of a Participant minor, and I hereby consent to his/her participation. I have read the foregoing release and indemnification agreement, and I hereby agree on behalf of myself and Participant to its terms. All donations are tax deductible, however registration fees may not be tax deductible due to IRS regulations – see your tax consultant for current IRS rules concerning funds exchanged for goods or services with a non-profit.
